Notices of Privacy Practices for Department of Health HIPAA Covered Programs
Notices of Privacy Practices are statements from each of the Department's HIPAA covered programs which describe how the program uses and discloses individually identifiable information maintained or collected by the program. The Notice also informs the reader of his/her rights with respect to the information and how the reader may exercise such rights, for example: the right to access the information, the right to request an amendment of the information, the right to restrict uses of the information, etc. The Notices are not forms to be completed, but rather are educational documents.
