Prevention Agenda - County Strategies and Partners Matrix
Information about each county's efforts to address Prevention Agenda priorities was compiled from the 2010-2013 community health assessments submitted in 2009 by local health departments. The information includes the identified priorities and focus areas; the collaborating hospitals, counties, and community partners; and the strategies being implemented to improve population health outcomes. The matrices can be viewed by priority area or by county.
