% of Pregnant Women in WIC who were pre-pregnancy overweight (BMI 26-29) The percent of pregnant women enrolled in the WIC program with a body mass index (BMI) between 26 and 29. BMI is defined as weight in kilograms divided by height in meters squared w/h2.
% of Pregnant Women in WIC who were pre-pregnancy very overweight (BMI > 29) The percent of pregnant women enrolled in the WIC program with a body mass index (BMI) greater than 29. BMI is defined as weight in kilograms divided by height in meters squared w/h2.

% of overweight children aged 2-4 participating in WIC The percent of children participating in the WIC program who are obese. Overweight is defined as between the 85 th and 95th percentile based on body mass index (BMI) for age. BMI is defined as weight in kilograms divided by height in meters squared w/h2.
% of obese children aged 2-4 participating in WIC The percent of children participating in the WIC program who are obese. Obese is defined as at or above the 95th percentile based on body mass index (BMI) for age. BMI is defined as weight in kilograms divided by height in meters squared w/h2.
% of children in WIC, 0-4 years, viewing TV <=2 hours per day The percent of children participating in the WIC program who watch television no more than 2 hours per day.

Source: Expanded BRFSS -All data relates to adults 18 years of age or older. The Expanded BRFSS is a telephone-based survey that collects information on self-reported health related behaviors. Between July 2002 and July 2003, the Expanded BRFSS was conducted in 38 localities (individual counties and county groupings) comprising the entire state. Expanded BRFSS-based indicators present the weighted percent and its 95% confidence interval. Statewide Expanded BRFSS data are available only on items contained in the "core" questionnaire. For county-selected optional items, the Expanded BRFSS results are compared to statewide BRFSS data.

% of adults eating 5 or more fruits or vegetables per day The percent of survey respondents (18+ years of age) that reported they ate at least five or more servings of fruits and vegetables in the past day.
Diabetes prevalence in adults The percent of survey respondents (18+ years of age) who reported they have been told by a health professional that they have diabetes.
% of adults with physician diagnosed angina, heart attack or stoke The percent of survey respondents (18+ years of age) who reported they have been told by a health professional that they have angina or have had a heart attack or stroke.
% of adults with physician diagnosed arthritis The percent of survey respondents (18+ years of age) who reported they have been told by a health professional that they have arthritis.
Mortality-Related Indicators Source: Vital Records NYSDOH Bureau of Biometrics and Health Statistics - Information is extracted from death certificates. When percentages are calculated, records with missing information for the indicator of interest are excluded from the calculation. Rates are calculated using population estimates provided by the US Census Bureau. ICD-10 codes are used for cause of death. Rates are age-adjusted based on the 2000 US Census population.

Cerebrovascular Disease (Stroke) Mortality (Rate per 100,000) age-adjusted The number of deaths due to cerebrovascular disease per 100,000 population. The ICD-10 codes used for cerebrovascular disease are: I60-I69.
Diabetes Mortality (Rate per 100,000) age-adjusted The number of deaths due to diabetes per 100,000 population. The ICD-10 codes used for diabetes are: E10-E14.
Hospitalization-Related Indicators Source: Statewide Planning and Research Cooperative System-SPARCS NYSDOH Bureau of Biometrics and Health Statistics- Data on hospitalizations are collected through the hospital inpatient discharge data system. Each hospitalization receives an ICD-9 code at discharge which indicates the primary reason for the hospitalization. Data are not available on events that did not result in a hospitalization, such as cases that were only treated in a hospital emergency room.

Cerebrovascular Disease (Stroke) Hospitalization (Rate per 10,000) age-adjusted The number of hospitalizations for cerebrovascular disease per 10,000 population. The ICD-9 codes used for cerebrovascular disease are: 430-438.
Diabetes Hospitalization (Rate per 10,000) age-adjusted The number of hospitalizations due to diabetes per 10,000 population. The ICD 9 code used for Diabetes is: 250. Only hospitalizations with diabetes as the primary diagnosis are included.
