New York State Community Health Data Set

The New York State Community Health Data Set consists of a series of tables, maps and graphs containing health statistics organized by County of residence. Several data sets were used to develop this information including:
- New York State birth, death, and fetal death files for mortality and natality rates
- Statewide Planning and Research Cooperative Systems (SPARCS) files, based on hospital discharges, for information on morbidity
- Department of Health disease registries, including cancer, AIDS/HIV, communicable disease, rabies, tuberculosis and sexually transmitted disease, for additional information on morbidity
- Program-based systems
Mortality and hospitalization based Indicators are presented as both crude and age-adjusted rates. The standard population used for adjustment was the 2000 United States population. Age adjustment allows for the comparison of rates between counties because they are less influenced by differences in county age distribution.
These Community Health Data are grouped into the following eighteen health-related sections. Each section contains a list of indicators contained in that section, an "about" file defining the listed indicators, describing the source(s) of data and identifying a contact person.

Information can be accessed by first, selecting one of the blue highlighted health related sections listed below. Within each section, there is a list of indicators with clickable blue highlighted labels that can be selected to access the tables, maps and charts. The following is a description of how data are presented within each labeled category:
- (Table + Trend): Indicators with this label are presented in a table with the latest 3 years of data and a clickable 10-year trend chart for each county. To access a trend chart, click on the blue highlighted county name in the table. A table and graph with 10 years of data will be displayed for the county and either Upstate (for counties outside of New York City) or New York City (for New York City counties). Clicking on the New York State Total will display a graph and chart with 10 years of data for New York State, Upstate and New York City.
- (Map): Indicators with this label are presented in two New York State maps with counties shaded based on the quartile distribution of the indicator of interest. The first map for each indicator presents crude rates and the 2nd, age-adjusted rates. There are also corresponding graphs displaying both crude and age-adjusted rates for all New York State counties.
- (Graph): Indicators with this label are presented in a ten year (where possible) bar graph with data for New York State. Data for these indicators are from statewide surveys that do not provide county level estimates. A few of these indicators do, however, include estimates for Upstate and New York City.
- (Table): Indicators with this label are presented in a table with the latest three years of data but do not have 10 year trend charts for the counties.
The tables can be saved as HTML or text for users who wish to work with the data with other software. The trend plots can be saved as files for insertion into word processing documents.
